CKA Volunteer Stipend Information Submission

CKA Volunteer Stipend & Expense Submission 

This form is used to collect the information required to process your volunteer stipend and/or expense reimbursement with the Canadian Kinesiology Alliance (CKA).

Please complete the form with your personal, payment, and (if applicable) expense details. For stipends over $500, tax information (including your SIN) is required in order to issue payment in accordance with CRA requirements.

If you are submitting an expense reimbursement, please:

• Complete and sign the CKA Expense Claim Form
  
• Combine all receipts into a single PDF file
  
• Upload both the signed form and receipt file
  

Incomplete submissions may delay processing.
